For GPs, neurologists, discharge planners and support coordinators: we take neurological referrals across the St George area — usually when someone needs more room, more time, or more specialised exercise prescription than a general clinic can give them.
What your patient gets
A one-to-one assessment with an ESSA Accredited Exercise Physiologist, a written program, scheduled reviews, and a purpose-built accessible facility — or home visits where travel is the barrier.
What you get
Reports back at assessment and at every review — to you, the specialist, or the plan manager. If we’re not the right service, we’ll say so and suggest who is.
Three ways referrals reach us.
GPs & specialists
Chronic disease management, DVA and workers comp referrals accepted. Request a callback and we will arrange the referral channel with your practice.
Support coordinators
NDIS registered to July 2029. Request a callback and we’ll take the participant’s goals and plan details, and respond within one business day.
Discharge planners
We pick up community rehab where inpatient programs finish. Request a callback and we’ll arrange how to receive the discharge summary.
